Your practice is working in partnership with the Dundee Carer Centre,134 Seagate, Dundee DD1 2HB. Telephone No. 01382 200422, to offer support to patients of any age who have carer responsibilities for someone who is ill, disabled or elderly.

If you are a Carer or the person being cared for please let the practice know so that we can update our records. Reception staff will provide you will a form for completion covering the relevant information including the question of consent for details to be entered against medical records.
